This article points to research indicating both size and distance matter. They used MRI on a few dozen women, which is still a small sample size (pun intended):

My study of this topic leads me to say that location, size, and level of sensitivity of the clitoris are the three main factors in whether a female reaches orgasm during PIV. Whether she has experienced an O apart from PIV is also understood to be a contributing factor, but that has nothing to do with characteristics of her clit.
